All Smiles Advantage Plan

All Smiles Dentistry announces our All Smiles Advantage Plan. This is a discount plan for our uninsured patients. Patients pay a yearly fee for maintenance appointments, as outlined below.

Adults

Includes: 2 cleanings, 2 exams, 4 BW X-rays, and one Pan or FMX every 3 – 5 years

Children

Includes: 2 cleanings, 2 exams, 2–4 BW X-rays, 1 Fluoride treatment, and one Pan every 3 – 5 years.

Perio Plan

Includes: 3 perio maintenance appointments, 2 exams, 4 BW X-rays, and one Pan or FMX every 3 – 5 years

PLUS - Membership in the All Smiles Advantage Plan allows you a 20% discount on prescribed treatment! Prescribed treatment includes: Fillings, crowns, bridges, dentures, partials, and oral surgery.

You must pay your portion in full at the time of service to qualify for the discount. All benefits must be used within a twelve-month period. Unused benefits cannot be carried into the next treatment year. Effective date: 01-01-2017.
